We appreciate you choosing Weiss Medical Associates for your neurologic needs. To ensure that you receive the best care possible, please familiarize yourself with our policies below:

Prescription Refills and Patient Messages
Please note that all prescription refills and patient messages will be addressed at the end of our patient day. It is best not to wait until the last day before your prescription runs out to call in a refill. When calling for a refill, be sure to leave your name, date of birth, prescription and instructions for taking, as well as the pharmacy name and phone number where you would like it to be called in. Please note that prescriptions called in after 3 pm will not be filled until the next business day.

Cancellations/No-Shows
All cancellations must be made at least 24 hours prior to scheduled appointment times. No-shows or cancellations made within 24 hours for office visits are subject to an office visit charge. No-shows or cancellations made within 24 hours for MRI appointments are subject to a fee of $250 per scan ordered.

Requests for Medical Records, CDs, Completion of Forms
Given the number of requests we receive for copies of medical records, copies of cds and completion of forms—and the time and overhead it takes for our administrative staff to complete these requests—we have made it our policy to charge a small fee for these services. Payment is due prior to receipt of all materials.

MRI cd - $10

Medical records - $1.00 per page, up to 25 pages ($0.50 for additional pages)
*no charge for records requested by other physician offices

Completion of Forms - $5-10 per form
(Disability forms, application for handicap parking sticker, etc.)


Billing Office

At Weiss Medical Associates, we want you to put your energy towards your health—not sorting through paperwork. We strive to make the billing and insurance process as seamless as possible, so you have less to worry about. Please refer to the information below to assist you with your billing needs.

To assist you in managing your care, our office will bill your insurance company directly for services rendered. Please be sure to provide any health insurance information you may have at the time of your visit to enable us to do so. While your insurance may cover all or part of your charges, it is ultimately up to you to ensure all parts of your bill have been covered—patients are responsible for any declined claims or balances that may remain after your insurance company has paid on them.

If you do not have health insurance—or if your insurance calls for co-pays, co-insurances, deductibles or fees for non-covered services—payment is expected at the time of service. To help make healthcare-related costs more manageable, Weiss Medical Associates gladly accepts Care Credit Patient Payment plans.

Our billing office is available Monday through Friday, from 8am to 4 pm MST. For billing questions related to services rendered from May 1, 2009 through today, please call (719) 478-3004. For questions related to charges incurred for services prior to May 1, please call 866-GMWEISS (866-469-3477). We are happy to review your account status with you.
